A Sick Note (MED3) is NOT required for the first 7 elapsed days of sickness – self-certify using the form SC2 https://assets.publishing.service.gov.uk/government/uploads/system/uploads/attachment_data/file/389298/sc2.pdf
If you require an extension to an existing Sick Note (MED3) created by a healthcare professional who is NOT part of the Edlesborough Surgery practice it will necessary to go back to that healthcare professional (e.g. hospital, etc.) to request that extension. Unfortunately the Surgery cannot assist you with this request
If you require a new or an extension of an Edlesborough Surgery created Sick Note (MED3) then contact the surgery – using the Edlesborough Surgery Digital Assistant (bottom right of this webpage)
